1) Complete all 3 documents at this link, and
2) Complete the 911 Supplement
3) Submit all parts to the Human Resources Office at 7 Court Street, Belmont, NY 14813.
4) Optional: Submit a copy directly to the 911 center staff at the Public Safety Complex,
4884 State Route 19, Belmont, NY 14813
Please note: Only applications that meet all minimum requirements and are
approved by Human Resources can be considered.
Submitting a copy directly to the 911 center only serves to alert the
911 staff to expect your official application after it is processed by
The Human Resources Office.
